Cognitive assessments: interdisciplinary team with medical evaluation by Dr. Tracy Hoffman of Hoffman Geriatric Services, neuropsychiatric testing by Dr. Cameron Goetz of Holy Family Memorial, and social services evaluation by a representative of the Alzheimer’s Association of Greater Wisconsin.  Evaluations done at clinic site at the Manitowoc Health and Rehabilitation Center, 2021 S. From Milwaukee:

Take I-43 North toward Green Bay. Take the US-151 exit, exit 149, toward WI-42. Turn left onto US-151 South/Calumet Avenue. Continue 1.5 miles and turn right onto South Alverno Road. The Health Care Center is the first building on the right side of the road.

From Green Bay:

Take I-43 South towards Milwaukee. Take the US-151/WI-42 South exit towards Manitowoc/Fond du Lac. Continue 1.5 miles and turn right onto South Alverno Road. The Health Care Center is the first building on the right.
